Download and fill out all the forms in the Registration Packet to bring with you to registration. The forms in the packet include: parental consent, medical release, certification record, and D.F.A. Policy Form. Be sure to also bring a copy of your child's birth certificate and latest report card to registration along with the registration fee. We will collect a copy of your child's final report card from you at the end of the school year.

| All of our forms are available for download by simply clicking the link and saving the form to your hard drive. Our forms are disseminated in Microsoft Word (.doc), Microsoft Excel (.xls), and Adobe (.pdf). If you do not have the appropriate software loaded on your machine, you will NOT be able to view and print the forms. However, if you do not have any of these applications, you may download the appropriate viewer and install it for free.
